That's a late 80s/90s bootleg of a handbill for one of Warhol's multimedia events sometimes involving the Velvet Underground. Probably some enterprising head shop put that together in their basement.

That documentary came out in the 90s, and he went from way underground obscurity to hipster hero/joke combination overnight.
